Job Description:

• Provide physical therapy services to home health patients
• Supervise Physical Therapy Assistants
• Evaluate therapeutic, rehabilitative, and functional status
• Instruct patients and families in the use of therapeutic appliances
• Assist in discharge planning and referral to other home health services

Qualifications:

• Bachelor's degree or higher in Allied Health Professions in Physical Therapy
• Current Ohio Physical Therapist license
• Preferred 2 years of community/home health experience
• Current CPR (AHA BLS) certificate
• Valid Ohio driver's license and motor vehicle insurance
