Job Description
- Deliver primary care services to elderly patients in diverse care facilities.
- Perform thorough medical assessments, create customized care plans, and manage chronic illnesses.
- Work collaboratively with a multidisciplinary team to ensure top-notch patient care.
- Utilize telemedicine tools for remote patient monitoring and continuity of care.
- Maintain accurate records in the Electronic Health Record (EHR) system.
Qualifications
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Maine.
- Previous experience in geriatrics, primary care, or a related field is preferred.
- Demonstrated ability to work independently and as part of a healthcare team.
- Proficiency with EHR systems and familiarity with telehealth technology.
- Strong clinical assessment and diagnostic skills.
