Job Description

• Deliver primary care services to elderly patients in assisted living, memory care, and long-term care facilities.
• Perform comprehensive medical assessments, develop personalized care plans, and effectively manage chronic conditions.
• Collaborate with a multidisciplinary team, including nurses, caregivers, and specialists, to ensure the best possible patient care.
• Utilize telemedicine and remote patient monitoring tools to ensure continuous and comprehensive healthcare.
• Maintain accurate and up-to-date documentation in the Electronic Health Record (EHR) system.

Qualifications

• Hold an active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Vermont.
• Prior experience in geriatrics, primary care, or a related field is preferred.
• Demonstrate the ability to work independently and collaboratively within a healthcare team.
• Proficiency with EHR systems and comfort using telehealth technology.
• Possess excellent clinical assessment and diagnostic skills.
