Job Description

• Deliver primary care services to elderly patients in diverse care facilities.
• Perform thorough medical assessments, create customized care plans, and manage chronic illnesses.
• Work collaboratively with a multidisciplinary team to ensure top-notch patient care.
• Utilize telemedicine tools for remote patient monitoring and continuity of care.
• Maintain accurate records in the Electronic Health Record (EHR) system.

Qualifications

• Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Maine.
• Previous experience in geriatrics, primary care, or a related field is preferred.
• Demonstrated ability to work independently and as part of a healthcare team.
• Proficiency with EHR systems and familiarity with telehealth technology.
• Strong clinical assessment and diagnostic skills.
