Job Description

  • Play a crucial role in providing primary care services to elderly patients in various assisted living, memory care, and long-term care settings.
  • Perform comprehensive medical assessments, develop personalized care plans, and effectively manage chronic conditions.
  • Collaborate closely with a diverse interdisciplinary team, including nurses, caregivers, and specialists, to ensure optimal patient care.
  • Leverage telemedicine and remote patient monitoring tools to deliver seamless continuity of care.
  • Maintain accurate and timely documentation using the Electronic Health Record (EHR) system.

Qualifications

  • Active Maine Nurse Practitioner (NP) or Physician Assistant (PA) license.
  • Prior experience in geriatrics, primary care, or a related field is highly desirable.
  • Demonstrated ability to work independently as well as collaboratively within a healthcare team.
  • Proficiency in working with EHR systems and comfortable utilizing telehealth technology.
  • Excellent clinical assessment and diagnostic skills.
