Job Description

• Provide physical therapy services to home health patients
• Supervise Physical Therapy Assistants
• Demonstrate knowledge of standard physical therapy practices in a home health setting
• Evaluate the therapeutic, rehabilitative, and functional status of patients
• Assess muscle strength, mobility, gait, and range of motion
• Develop and implement individualized treatment plans
• Instruct patients, families, and caregivers on the use and care of therapeutic appliances
• Report patient's reaction to treatment or changes in condition to the physician
• Complete medication reconciliation when necessary
• Document patient visits and progress within specified timeframes
• Collaborate with other healthcare professionals in case conferences
• Provide training on the use of prosthetic devices, if applicable
• Identify additional home health services needed and make appropriate referrals
• Prepare clinical and progress summaries for patient discharge planning

Qualifications

• Bachelor's degree or higher in Science in Allied Health Professions in Physical Therapy from an accredited program
• Current license as a Physical Therapist in the State of Ohio
• Preferred two years of community/home health experience as a Physical Therapist
• Current CPR (AHA BLS) certificate
• Valid Ohio driver's license and motor vehicle insurance
