Job Description:
- Provide physical therapy services to home health patients
- Supervise Physical Therapy Assistants
- Ensure knowledge of standard physical therapy practices in a home health setting
- Assess and evaluate therapeutic, rehabilitative, and functional status
- Participate in the development of comprehensive care plans
- Evaluate the patient's home environment
- Assess muscle strength, mobility, gait, and range of motion
- Administer direct physical therapy treatment
- Instruct patients, families, and caregivers on the use and care of therapeutic appliances
- Determine priority needs for physical therapy
- Report patient's reaction to treatment or changes in condition to physician
- Complete medication reconciliation for therapy-only patients as needed
- Ensure timely completion of OASIS charting
- Initiate physical therapy program and provide instructions to other personnel and family members
- Complete all charting within 72 hours of each visit
- Train patients in the use of prosthetic devices if necessary
- Identify patient and family/caregiver needs for other home health services and make appropriate referrals
- Prepare and submit clinical and progress summaries based on goals attainment
- Participate in discharge planning
Qualifications:
- Bachelor's degree or higher in Science in Allied Health Professions in Physical Therapy from an accredited program
- Current Physical Therapist license in the State of Ohio
- Preferred two years of community/home health experience
- Current CPR (AHA BLS) certification required
- Valid Ohio driver's license with motor vehicle insurance
