Job Description:

• Provide physical therapy services to home health patients
• Supervise Physical Therapy Assistants
• Ensure knowledge of standard physical therapy practices in a home health setting
• Assess and evaluate therapeutic, rehabilitative, and functional status
• Participate in the development of comprehensive care plans
• Evaluate the patient's home environment
• Assess muscle strength, mobility, gait, and range of motion
• Administer direct physical therapy treatment
• Instruct patients, families, and caregivers on the use and care of therapeutic appliances
• Determine priority needs for physical therapy
• Report patient's reaction to treatment or changes in condition to physician
• Complete medication reconciliation for therapy-only patients as needed
• Ensure timely completion of OASIS charting
• Initiate physical therapy program and provide instructions to other personnel and family members
• Complete all charting within 72 hours of each visit
• Train patients in the use of prosthetic devices if necessary
• Identify patient and family/caregiver needs for other home health services and make appropriate referrals
• Prepare and submit clinical and progress summaries based on goals attainment
• Participate in discharge planning

Qualifications:

• Bachelor's degree or higher in Science in Allied Health Professions in Physical Therapy from an accredited program
• Current Physical Therapist license in the State of Ohio
• Preferred two years of community/home health experience
• Current CPR (AHA BLS) certification required
• Valid Ohio driver's license with motor vehicle insurance
